MAKE AN APPOINTMENT

To begin individual therapy with me:

  1. Call me at 978-378-3533, so we can have a brief conversation about what you are experiencing and need.
  2. If appropriate, we will make an appointment for an initial assessment appointment which lasts 60 minutes, during which we will talk at length about your symptoms and background/history and begin to develop a plan for treatment.
  3. Before your first appointment, I will send you a link to my secure Client Portal. Here, you will fill out an online intake form, providing information about yourself and your health, and read/sign my privacy, informed consent and practice policies. You are welcome to ask any questions or address concerns with me about this information or if you have trouble accessing the Client Portal.
  4. You will then have ongoing access to my Client Portal to make/change appointments, pay/view bills, securely store your credit card information and send/receive messages.

To begin ketamine-assisted therapy (KAP) with me: 

  1. Please complete this form.
  2. Once I have received your completed pre-screening form, I will get in touch to schedule a free, 15-minute phone conversation to discuss your eligibility and next steps.
  3. If you have a primary therapist and we both determine that KAP appears to be a safe and appropriate treatment option, I will ask you to sign a release of information form so that I can contact your primary therapist to collaborate and develop a collaborative treatment plan.
  4. You will review and sign my KAP informed consent and practice policies and complete the intake form. We will tentatively schedule a dates for preparation, medicine session and integration.
  5. I will facilitate a referral to either Journey Clinical (for residents of MA) or Shauna Sedler, NP (for residents of NH). Journey Clinical will contact you via email to arrange your comprehensive medical assessment and I will give you Shauna’s email to contact her directly about arranging your assessment. Journey Clinical appointments are done virtually, while Shauna conducts in-person consultations in Biddeford, ME. During the evaluation, they will conduct a comprehensive medical assessment to ensure safety. Additionally, they will discuss treatment planning, risk reduction and address any medical inquiries regarding ketamine, dosing, or other relevant medical concerns. It’s important to note that this assessment doesn’t guarantee eligibility for participation, but we do our best rule out any potential disqualifying circumstances or conditions beforehand.
  6. Once your medical evaluation is complete, we can confirm the dates for your ketamine-assisted therapy and you will be charged your non-refundable deposit.
  7. Preparation is crucial for a successful ketamine experience. This involves a 50-minute telehealth session or an hour group Zoom session dedicated to preparing yourself physically, mentally and spiritually for the experience. I will provide detailed guidelines and instructions for preparation, tools to navigate the journey and guidance on self-care afterward.

Due to privacy and HIPAA rules, I cannot communicate with you via Facebook or text. We will communicate via phone and my secure Client Portal.
